Please find enclosed the following items comprising the DS-230 packet. I would kindly request that originals of pertinent documents be returned to my spouse at the time of the interview in US Embassy in Manila (my spouse can provide photocopies for your official records, if needed):

• Cover Sheet from NVC
• DS-230 Part 1 completed and signed by beneficiary
• DS-230 Part 2 completed by beneficiary – unsigned, as of yet, per instructions
•  Two (2) Passport Size Photographs of Beneficiary (NVC Case Number, beneficiary’s full name, and date of birth are written on back of both)
• Photocopy of the Biographic Data Page of Beneficiary’s Passport
•  Authenticated / Certified Birth Certificate of beneficiary (National Statistic Office) and a copy
•  Authenticated / Certified Marriage Certificate  (National Statistic Office) and a copy
• Original/Certified Police Clearance Certificate (National Bureau of Investigation) and a copy
